In order to edit and submit your forms from your Android you will first need to download the Adobe Acrobat Reader app. This is a free app that just functions as a pdf viewer. Open the app and create an account. Again all this is free.
Once you have the app downloaded, open the form you need in the Adobe Acrobat Reader app. Fill out the form and then hit the share button. In the box, enter theclinic@casscohealth.org and share your completed form with our clinic email address.
